My name is Kevin M*****. I was baptised into the Catholic Church in Clones on 26/06/19**  by my parents when I was a few weeks old.

Obviously I had no choice in this at the time.
 
In light of the Ryan Report I can no longer be part of this truly wicked, despicable and evil organisation.  My conscience and moral judgement forces me to abandon any connection with the Catholic Church, I am no longer a Catholic.
 
I request that you amend the baptismal record to reflect my wish. In so doing I will not be counted as a member of the Catholic congregation.
 
I retain Christian beliefs and following research I have decided to join the Anglican faith here in my adopted home of Wicklow town.
 
A reply to confirm that you will take this action would be appreciated.
